Immune system activity refers to the collective physiological processes and responses orchestrated by the innate and adaptive immune systems to protect the host from pathogens, toxins, and malignant cells (NIH, 2023). It is not a single molecular target but rather a complex network of cellular interactions and signaling pathways involving various leukocytes, cytokines, and lymphoid organs (StatPearls, 2023). In a healthy state, immune activity maintains homeostasis through a balance of activation and regulation; however, dysregulation can lead to significant pathology. Overactive immune responses are central to autoimmune diseases and chronic inflammation, while suppressed activity increases susceptibility to infections and cancer (Janeway's Immunobiology, 2016). Pharmacological modulation of immune system activity is a cornerstone of modern medicine, utilizing immunosuppressants to treat transplant rejection and autoimmunity, or immunostimulants and checkpoint inhibitors to enhance anti-tumor responses. Because "immune system activity" encompasses thousands of distinct proteins and pathways, drug discovery focuses on specific molecular targets within this system, such as cytokines, G protein-coupled receptors, or kinases, rather than the activity as a monolithic entity.
Modulation of immune system activity occurs through diverse mechanisms including the inhibition of pro-inflammatory cytokines, blockade of immune checkpoints, depletion of specific lymphocyte populations, or agonism of pattern recognition receptors (StatPearls, 2023).
