Immunomodulatory mediators are a broad class of endogenous signaling molecules, such as cytokines, chemokines, and growth factors, that regulate the activity and duration of the immune response (StatPearls, 2023). These molecules are essential for maintaining immune homeostasis and coordinating the body's defense against pathogens and cellular abnormalities (NIH, 2022). Dysregulation of these mediators is a primary driver in the pathogenesis of autoimmune diseases, chronic inflammation, and the progression of various cancers (PubMed, 2021). In clinical practice, these mediators are targeted by a wide array of drugs, including monoclonal antibodies that neutralize pro-inflammatory cytokines or recombinant proteins that mimic natural regulatory signals. For instance, TNF-alpha inhibitors are used to treat rheumatoid arthritis, while checkpoint inhibitors modulate mediators to enhance anti-tumor immunity (NCBI, 2023). Because many of these mediators have pleiotropic effects, therapeutic strategies must balance the need for efficacy with the risk of systemic side effects like immunosuppression.
