ERBB2 (HER2) and ESR1 (Estrogen Receptor alpha) are distinct but frequently co-evaluated molecular targets in oncology, particularly in breast cancer. ERBB2 is a receptor tyrosine kinase that, upon activation or overexpression, triggers potent intracellular signaling cascades like the PI3K/AKT and MAPK pathways to promote cell growth and survival (UniProt P04626). ESR1 is a nuclear hormone receptor that acts as a transcription factor, regulating gene expression in response to estrogen to drive cellular proliferation in reproductive tissues (UniProt P03372). In clinical practice, the expression status of these two proteins defines major therapeutic categories: HER2-positive and ER-positive breast cancers. Bidirectional crosstalk between ERBB2 and ESR1 is a well-documented mechanism of drug resistance, where signaling from one pathway can bypass the inhibition of the other (PubMed: 24562530). Consequently, dual targeting or sequential treatment strategies are often employed to manage advanced disease and prevent recurrence (NIH: NBK538259).
ERBB2-targeted drugs function by blocking the extracellular dimerization of the receptor, inhibiting its intracellular tyrosine kinase activity, or delivering cytotoxic payloads via antibody-drug conjugates. ESR1-targeted therapies act as selective estrogen receptor modulators (SERMs), selective estrogen receptor degraders (SERDs), or by lowering systemic estrogen levels to prevent receptor activation.
