Overview

Key proteins include filaggrin, loricrin, and involucrin, which are cross-linked by transglutaminases to form the cornified envelope, as well as keratins and tight junction proteins like claudins that seal the paracellular space. These elements collectively prevent transepidermal water loss and provide a robust defense against environmental stressors, allergens, and microbial pathogens. Dysregulation of these pathways, often driven by genetic mutations (e.g., FLG) or inflammatory cytokines (e.g., IL-4, IL-13, IL-17), is a primary driver of dermatological diseases such as atopic dermatitis, psoriasis, and ichthyosis. Therapeutic interventions aim to restore barrier function through the application of lipid-replenishing emollients, the use of biologics that inhibit barrier-degrading cytokines, and the development of agents that upregulate structural protein expression.
