The synovial membrane is the specialized connective tissue that lines the inner surface of joint capsules, excluding the articulating surfaces of cartilage (Firestein & McInnes, 2017). It consists of a lining layer (intima) containing fibroblast-like synoviocytes and macrophage-like synoviocytes (synovial macrophages), and a sublining layer rich in blood vessels and nerves (Alivernini et al., 2020). Synovial macrophages play a dual role: they maintain joint homeostasis through debris clearance and anti-inflammatory signaling, but they also serve as primary drivers of chronic inflammation in diseases like rheumatoid arthritis (Tu et al., 2020). In pathological states, these cells become highly activated, secreting potent pro-inflammatory cytokines such as TNF-alpha and IL-6, which promote bone erosion and cartilage degradation (Udalova et al., 2016). While the synovial membrane is a tissue rather than a single molecular target, it is the focal point for many therapeutic strategies aimed at dampening the immune response in the joint. Modern therapies, such as TNF inhibitors and JAK inhibitors, target the specific signaling molecules or receptors expressed by these cells to restore joint function and prevent structural damage (Firestein & McInnes, 2017). Understanding the heterogeneity of synovial macrophages is currently a major focus for developing precision medicine approaches in rheumatology (Alivernini et al., 2020).
