Overview

Tumor necrosis factor receptor superfamily member 11B (TNFRSF11B), commonly known as osteoprotegerin (OPG), is a secreted glycoprotein and member of the tumor necrosis factor receptor superfamily. It serves as a **decoy receptor** for receptor activator of nuclear factor kappa-B ligand (RANKL) and TNF-related apoptosis-inducing ligand (TRAIL), inhibiting osteoclastogenesis (formation of bone-resorbing cells) and TRAIL-induced apoptosis. OPG is primarily produced by osteoblasts, endothelial cells, and other tissues, and plays central roles in bone remodeling, immunoregulation, vascular biology, and multiple disease processes including osteoporosis, cancer (notably in bone metastases), autoimmune, and cardiovascular disorders. OPG’s biological activity is modulated by various signaling pathways, including estrogen, Wnt/β-catenin, and MAPK. Clinically, it is a potential biomarker for bone disease and cardiovascular risk, and its pharmacological modulation underpins the action of therapeutic agents like denosumab[1][2][3][4][6].

Mechanism of action

Acting as a decoy receptor for RANKL, preventing RANK-RANKL signaling, thus reducing osteoclast formation and bone resorption[1][2][3] Binding to TRAIL, preventing TRAIL-induced apoptosis[1][3] Binding other ligands (von Willebrand factor, syndecan-1, glycosaminoglycans, factor VIII-vWF complex) possibly modulating related signaling[1][3]
