Overview

Type 1 T helper (Th1) cells are a specialized subset of CD4+ T lymphocytes that play a central role in the adaptive immune system by coordinating cell-mediated immunity [1.3.2, 1.4.2]. They are primarily characterized by the production of pro-inflammatory cytokines, most notably interferon-gamma (IFN-gamma), interleukin-2 (IL-2), and tumor necrosis factor-alpha (TNF-alpha) [1.1.2, 1.3.2]. Th1 differentiation is driven by the cytokine IL-12 and the master transcription factor T-bet, and these cells are essential for the host's defense against intracellular pathogens such as viruses and certain bacteria like Mycobacterium tuberculosis [1.3.2, 1.4.2]. However, excessive or dysregulated Th1 activity is a major driver of various autoimmune and chronic inflammatory diseases, including rheumatoid arthritis, multiple sclerosis, and Crohn's disease [1.1.2, 1.3.1]. Therapeutic interventions targeting the Th1 pathway include biologics that neutralize Th1-derived cytokines (e.g., TNF-alpha inhibitors), small molecules that block downstream signaling (e.g., JAK inhibitors), and agents that prevent Th1 differentiation or trafficking [1.3.1, 1.4.1].

Mechanism of action

Drugs targeting the Th1 response typically act by neutralizing effector cytokines (e.g., TNF-alpha inhibitors), blocking cytokines required for Th1 differentiation (e.g., IL-12/23 inhibitors), or inhibiting intracellular signaling pathways like JAK-STAT that mediate Th1 cytokine effects [1.3.1].
