Vesicular stomatitis virus (VSV)-mediated oncolysis is a cancer therapy approach that utilizes VSV, an RNA virus, to selectively infect and destroy cancer cells. The virus exploits defects in the type I interferon (IFN) signaling pathway, which are common in tumors, allowing preferential replication in malignant cells while sparing normal tissue. The oncolytic mechanism involves direct cell lysis, vascular shutdown, and immune stimulation. Engineering strategies focus on enhancing safety, selectivity, and immunogenicity. Resistance can occur due to intact IFN responses or poor viral attachment, and may be overcome by combination therapies.
