Overview

The combination of 5-aminosalicylic acid (5-ASA) and adalimumab is used primarily for treating inflammatory bowel diseases, particularly ulcerative colitis and Crohn's disease. This combination therapy approach is sometimes employed when patients have failed to respond adequately to other treatments or when transitioning between therapies. 5-Aminosalicylic acid (also known as mesalamine) works by blocking production of prostaglandins and leukotrienes and has other beneficial effects on the inflammatory cascade[5]. It is active intraluminally in the gastrointestinal tract and is formulated for delayed absorption when given orally. Adalimumab is a tumor necrosis factor (TNF) blocker that reduces inflammation by inhibiting TNF, resulting in down-regulation of the immune-mediated inflammatory response associated with conditions like Crohn's disease and ulcerative colitis[8]. There are documented cases where methotrexate was discontinued and replaced with 5-aminosalicylic acid while maintaining adalimumab therapy, with no recurrence observed[1]. However, there have also been reports of potential adverse effects with this combination, including a case of drug-induced hepatotoxicity that resolved after discontinuation of both medications[8]. Clinical studies have explored various combination therapies involving adalimumab, including combinations with intensive granulocyte and monocyte adsorptive apheresis (GMA) for refractory ulcerative colitis[2]. Some patients receiving adalimumab therapy may also receive additional treatments such as azathioprine when there is insufficient response to the initial therapy[2]. For ulcerative colitis specifically, adalimumab is indicated for moderately to severely active disease in adults and pediatric patients (age requirements vary by brand), though effectiveness has not been established in patients who have lost response to or were intolerant to TNF blockers[6][7]. This combination appears to be generally well-tolerated, though monitoring for potential adverse effects is recommended, particularly given the reported case of hepatotoxicity with concurrent use[8].
