This is a combination therapy consisting of four monoclonal antibody biologics—adalimumab, infliximab, ustekinumab, and vedolizumab—used in the management of inflammatory bowel diseases (IBD) such as Crohn's disease and ulcerative colitis. Each component targets a different immune pathway involved in IBD pathogenesis: - Adalimumab and infliximab are anti-tumor necrosis factor alpha (anti-TNFα) antibodies that inhibit TNFα-mediated inflammation. - Ustekinumab targets the p40 subunit shared by interleukin-12 (IL-12) and interleukin-23 (IL-23), modulating T-cell mediated immune responses. - Vedolizumab is an integrin α4β7 antagonist that blocks lymphocyte trafficking to the gut. Combination or dual biologic therapy has been explored for patients with refractory or complicated IBD who have failed multiple lines of advanced therapies. While combinations such as infliximab plus ustekinumab or vedolizumab plus ustekinumab have been studied in clinical trials and case series, there is limited evidence on quadruple biologic combinations due to concerns about safety, immunosuppression risk, and lack of robust efficacy data[1][2][4]. The rationale for combining these agents is to achieve superior disease control by targeting multiple inflammatory pathways simultaneously.
