A triple combination therapy consisting of vedolizumab (a gut-selective α4β7 integrin antagonist), adalimumab (a TNF inhibitor), and methotrexate (an immunosuppressant). This combination was studied in the EXPLORER trial for treating biologic-naïve patients with newly diagnosed, moderate to high-risk Crohn's disease. The regimen includes vedolizumab 300 mg IV on day 1, weeks 2 and 6, then every 8 weeks; adalimumab 160 mg SC on day 2, 80 mg at week 2, then 40 mg every 2 weeks; and methotrexate 15 mg orally weekly.
