Overview

This is a combination regimen consisting of four antimicrobial agents—amphotericin B, cefotaxime, ciprofloxacin, and metronidazole. Each drug has a distinct mechanism of action and spectrum of activity: - **Amphotericin B** is an antifungal agent that binds to ergosterol in fungal cell membranes, creating pores that lead to cell death. It is used for severe systemic fungal infections[1]. - **Cefotaxime** is a third-generation cephalosporin antibiotic with broad-spectrum activity against Gram-positive and Gram-negative bacteria by inhibiting bacterial cell wall synthesis[3]. - **Ciprofloxacin** is a fluoroquinolone antibiotic that inhibits bacterial DNA gyrase and topoisomerase IV, preventing DNA replication in susceptible bacteria[9]. - **Metronidazole** is an antiprotozoal and antibacterial agent effective against anaerobic bacteria and certain parasites; it disrupts DNA synthesis after reduction within the microorganism[6]. This combination would provide very broad coverage against fungi (especially yeasts), aerobic Gram-positive/negative bacteria (including many Enterobacteriaceae), anaerobes (including Bacteroides species), as well as some protozoa. Such regimens may be considered in cases of severe mixed infections or empiric therapy for life-threatening intra-abdominal or immunocompromised host infections where polymicrobial etiology—including resistant organisms—is suspected.
