Overview

An anti-CD3/anti-CD19 bispecific T-cell engager (BiTE) is a recombinant bispecific antibody designed to redirect the cytotoxic activity of T cells against CD19-expressing B cells. The molecule typically consists of two single-chain variable fragments (scFvs) linked in tandem: one scFv binds to the CD3 epsilon subunit of the T-cell receptor complex, while the other binds to CD19, a pan-B-cell marker. By cross-linking these two antigens, the BiTE induces T-cell activation, proliferation, and the formation of a cytolytic synapse, leading to the targeted destruction of B-lineage cells through the release of perforin and granzymes. This approach is highly effective in treating B-cell malignancies, as it operates independently of MHC-I expression. Blinatumomab (Blincyto) is the first-in-class therapeutic utilizing this platform and is approved for the treatment of B-cell precursor acute lymphoblastic leukemia (B-ALL).
