Overview

**Baricitinib + fluconazole** is a combination of two small molecule drugs: baricitinib, a Janus kinase (JAK) inhibitor, and fluconazole, a triazole antifungal. \n- **Baricitinib** selectively and reversibly inhibits Janus kinase 1 (JAK1) and Janus kinase 2 (JAK2), modulating cytokine-driven signaling involved in immune function and inflammation. It is primarily used to treat moderate to severe rheumatoid arthritis, atopic dermatitis, alopecia areata, and has approval for COVID-19 in specific hospitalized patient populations[4][1][3].\n- **Fluconazole** inhibits fungal cytochrome P450 enzymes, primarily 14α-demethylase (CYP51), thereby interfering with the synthesis of ergosterol, an essential component of fungal cell membranes. It is widely used for the treatment and prophylaxis of fungal infections, especially Candida and Cryptococcus species.\n\nNo branded combination product of baricitinib + fluconazole is approved or marketed, and limited or no clinical data exists on deliberate combined use. Available pharmacokinetic data indicate that coadministration of baricitinib with fluconazole (a moderate CYP3A/CYP2C9/CYP2C19 inhibitor) does **not result in clinically meaningful changes** to baricitinib exposure[2][1]. The combination should only be considered under special circumstances, with continuous monitoring due to immunosuppressive properties of baricitinib and the interaction profile of fluconazole.
