This is a hypothetical combination of four Janus kinase (JAK) inhibitors: baricitinib, peficitinib, tofacitinib, and upadacitinib. Each is an oral small molecule inhibitor targeting different members of the JAK family involved in cytokine signaling pathways. These drugs are primarily used for the treatment of rheumatoid arthritis and other immune-mediated inflammatory diseases. Baricitinib preferentially inhibits JAK1 and JAK2; peficitinib predominantly inhibits JAK3 but also affects JAK1, JAK2, and TYK2; tofacitinib mainly targets JAK1 and JAK3 with some activity on JAK2; upadacitinib is a selective inhibitor of JAK1 with some effect on JAK2 at higher doses[2][3][5]. All four agents modulate immune responses by interfering with cytokine receptor signal transduction.
