CD20-CD3 bispecific antibodies (BsAbs) are a class of T-cell engaging immunotherapies designed to treat B-cell malignancies, such as large B-cell lymphoma (LBCL) and follicular lymphoma. These molecules possess dual binding specificity: one arm targets the CD20 antigen, which is highly expressed on the surface of mature B-cells and most B-cell lymphomas, while the other arm targets the CD3 subunit of the T-cell receptor complex. By bridging these two cell types, the BsAb facilitates the formation of an immunological synapse, leading to the activation, proliferation, and directed cytotoxic activity of T-cells against the CD20-positive tumor cells, independent of major histocompatibility complex (MHC) recognition. Several agents in this class, including mosunetuzumab, glofitamab, and epcoritamab, have received regulatory approval for various lymphoma indications.
