Overview

This is a combination drug consisting of four active pharmaceutical ingredients:

  • Ciprofloxacin is a second-generation fluoroquinolone antibiotic that inhibits bacterial DNA gyrase and topoisomerase IV, leading to inhibition of DNA replication and cell death. It is primarily used for the treatment of various susceptible bacterial infections, including skin, bone, joint, respiratory tract, urinary tract infections, and others.
  • Fluconazole is a triazole antifungal agent that inhibits fungal cytochrome P450 enzyme 14α-demethylase. This disrupts ergosterol synthesis in fungal cell membranes, leading to increased membrane permeability and cell death. It is used for systemic and superficial fungal infections such as candidiasis.
  • Neomycin is an aminoglycoside antibiotic that binds to the 30S ribosomal subunit of bacteria, inhibiting protein synthesis. It has broad-spectrum activity against many Gram-negative bacteria but limited use systemically due to toxicity; it’s often used topically or orally for bowel decontamination.
  • Vancomycin is a glycopeptide antibiotic that inhibits bacterial cell wall synthesis by binding tightly to D-Ala-D-Ala terminus of cell wall precursor units. It’s mainly indicated for serious Gram-positive infections including those caused by methicillin-resistant Staphylococcus aureus (MRSA) and Clostridium difficile-associated diarrhea when given orally.

This combination would provide broad-spectrum antibacterial coverage (including both Gram-positive and Gram-negative organisms), antifungal activity (against Candida species), as well as topical or gastrointestinal decontamination via neomycin.
