Overview

The combination of ciprofloxacin and infliximab is a therapeutic approach used primarily for treating perianal Crohn's disease. This combination therapy has shown promising results in clinical studies, with evidence suggesting it may be more effective than infliximab alone in treating perianal fistulas in Crohn's disease patients. A double-blind placebo-controlled study evaluated the effect of combined ciprofloxacin and infliximab in perianal Crohn's disease. In this study, patients were randomly assigned to receive either 500-mg ciprofloxacin twice daily or a placebo for 12 weeks, with all patients receiving 5-mg/kg infliximab at weeks 6, 8, and 12. The results showed that at week 18, the response rate was 73% in the ciprofloxacin group compared to 39% in the placebo group. The Perianal Disease Activity Index score significantly improved (P = 0.008) only in the ciprofloxacin group, providing further evidence of the combination's effectiveness. The combination works through complementary mechanisms: Ciprofloxacin is a fluoroquinolone antibiotic, and Infliximab is a monoclonal antibody that targets tumor necrosis factor alpha (TNF-α), a key inflammatory mediator in Crohn's disease. The combination appears to provide enhanced efficacy through both antimicrobial and anti-inflammatory effects. Similar combination approaches with other anti-TNF agents, like adalimumab with ciprofloxacin, have also shown superiority to monotherapy.
