Overview

This is a combination regimen consisting of four drugs—clarithromycin, clofazimine, infliximab, and rifabutin. Each component has a distinct mechanism of action: - **Clarithromycin** is a macrolide antibiotic that inhibits bacterial protein synthesis by binding to the 50S ribosomal subunit. - **Clofazimine** is an antimycobacterial agent with anti-inflammatory properties; it binds to mycobacterial DNA and disrupts replication and transcription. - **Infliximab** is a chimeric monoclonal antibody that targets tumor necrosis factor alpha (TNFα), inhibiting its pro-inflammatory activity[3][7]. - **Rifabutin** is an antibiotic in the rifamycin class that inhibits DNA-dependent RNA polymerase in bacteria, suppressing RNA synthesis and leading to cell death[6][8][10]. This combination may be considered for complex infections involving mycobacteria or for investigational use in severe inflammatory or infectious diseases where both immunomodulation (via infliximab) and broad-spectrum antimycobacterial coverage are required.
