conditioning regimen C (Center for International Blood and Marrow Transplant Research)

Conditioning regimen C (more accurately referred to as Regimen C in the ACCELERATE platform protocol) is an experimental graft-versus-host disease (GVHD) prophylaxis regimen being evaluated by the Center for International Blood and Marrow Transplant Research (CIBMTR). It consists of a backbone of post-transplant cyclophosphamide (PTCy), tacrolimus, and mycophenolate mofetil (MMF), with the addition of the JAK1/2 inhibitor ruxolitinib. This regimen is specifically studied in patients with hematologic malignancies undergoing mismatched unrelated donor (MMUD) peripheral blood stem cell transplantation. The goal of adding ruxolitinib to the PTCy-based platform is to further reduce the incidence and severity of acute and chronic GVHD by modulating cytokine signaling and T-cell activation. Although the term "conditioning" typically refers to pre-transplant cytoreduction, in the context of this trial, Regimen C is a post-transplant prophylaxis combination.
