This combination is known as CVPP (Cyclophosphamide, Vinblastine, Procarbazine, and Prednisone), a chemotherapy regimen used primarily for treating advanced Hodgkin's disease[1][4]. The regimen was used in clinical settings from at least the 1970s, with long-term follow-up studies reporting outcomes after more than a decade of observation[1]. ## Dosing Information The typical dosing schedule for CVPP involves: - Cyclophosphamide: 600 mg/m² intravenously on day 1 - Vinblastine: 6 mg/m² intravenously on day 1 - Procarbazine: 100 mg/m²/day orally on days 1 through 14 - Prednisone: administered throughout the treatment cycle[3] This combination is administered in monthly cycles. ## Efficacy Studies have shown that CVPP can be effective in treating advanced Hodgkin's disease: - In a study of 38 patients treated from 1970 to 1973, 28 patients (74%) achieved complete response[1] - Of those complete responders, 17 (61%) survived more than 10 years from the start of chemotherapy[1] - 12 patients (43% of complete responders) remained continuously disease-free for 12.8 to 15.3 years after completing induction chemotherapy[1] - All relapses occurred within 5.5 years of completing induction chemotherapy[1] ## Long-term Complications Long-term follow-up has identified several late complications associated with CVPP treatment: - Sterility - Aseptic osteonecrosis - Severe pulmonary fibrosis - Chronic uveitis - Second neoplasms (14% of complete responders), including: - Acute myelogenous leukemia - Non-Hodgkin lymphoma - Small cell carcinoma of the lung[1] All second malignancies were fatal and developed 5-13 years after initiation of induction chemotherapy[1]. ## Alternative Regimens CVPP has been studied as an alternative to MOPP (nitrogen mustard, vincristine, procarbazine, and prednisone), with data confirming that cure is possible with this alternative regimen[1]. Another variation is the C-MOPP regimen, which uses cyclophosphamide instead of nitrogen mustard but includes vincristine rather than vinblastine. This has been studied in alternating combination with ABVd (adriamycin, bleomycin, vinblastine, dacarbazine) for Japanese patients with advanced Hodgkin's disease[2]. The C-MOPP/ABVd alternating regimen showed: - 92.5% response rate with 83.6% complete response - 84.8% overall 5-year survival rate - 72.8% progression-free 4-year survival rate[2]
