**Elritercept + ruxolitinib** is an investigational combination therapy studied for the treatment of myelofibrosis with anemia. Elritercept (KER-050) is a recombinant fusion protein that consists of a modified activin receptor type-2A extracellular domain fused to an IgG1 Fc, designed to inhibit Activin A/B and other select TGF-β superfamily ligands (including GDF8/11), thus promoting hematopoiesis and improving anemia. Ruxolitinib is an oral small-molecule inhibitor of Janus kinase 1 and 2 (JAK1/2), widely used to reduce spleen size and symptoms in myelofibrosis but often associated with cytopenias. The combination aims to address both ineffective hematopoiesis and the cytopenias (especially anemia) that can be caused or worsened by JAK inhibition. In the RESTORE phase 2 trial, the combination has demonstrated improvements in hemoglobin, transfusion independence, platelet counts, spleen volume, and symptom burden in myelofibrosis patients with anemia.
