fingolimod + venlafaxine

Fingolimod + venlafaxine is a combination therapy consisting of fingolimod, an oral sphingosine 1-phosphate receptor modulator primarily used for the treatment of relapsing-remitting multiple sclerosis (RRMS), and venlafaxine, a serotonin-norepinephrine reuptake inhibitor (SNRI) antidepressant. This combination has been studied in patients with RRMS who also have mild-to-moderate depression. The rationale for this combination is to address both MS disease activity and comorbid depressive symptoms, which are common in this population. Mechanistically, fingolimod acts by sequestering lymphocytes in lymph nodes via modulation of sphingosine 1-phosphate receptors, thereby reducing neuroinflammation associated with MS. Venlafaxine increases synaptic concentrations of serotonin and norepinephrine by inhibiting their reuptake into presynaptic neurons, alleviating depressive symptoms[1][2][3]. Clinical studies have shown that the combination does not result in unexpected safety signals compared to monotherapy; patient-reported outcomes such as quality of life, fatigue, disability, and depression remained stable or improved during treatment[2][4]. However, caution is advised due to potential pharmacodynamic interactions such as QT prolongation risk[5].
