FT2109 is an orally bioavailable small molecule inhibitor of soluble tumor necrosis factor (sTNF) signaling, currently in Phase 1 development by Forward Therapeutics for the treatment of chronic inflammatory and autoimmune diseases. Developed using the company's SWIFT discovery engine, FT2109 belongs to a novel class of TNF signaling inhibitors (TNFsi) that selectively target the soluble form of TNF while sparing the membrane-bound form (mTNF). The mechanism involves binding to the sTNF trimer and disrupting its symmetry, which prevents it from effectively signaling through the pro-inflammatory TNF receptor 1 (TNFR1). By preserving mTNF-TNFR2 signaling, which is essential for immune regulation and tissue homeostasis, FT2109 aims to provide the therapeutic benefits of traditional TNF blockers (such as monoclonal antibodies) with a significantly improved safety profile, potentially avoiding the systemic immunosuppression and associated boxed warnings of non-selective TNF antagonists.
