Gecacitinib + donafenib + PD-1 inhibitor is a combination therapy regimen being evaluated for the treatment of unresectable hepatocellular carcinoma (HCC). The regimen consists of gecacitinib, a selective Janus kinase 1 (JAK1) inhibitor; donafenib, a deuterated multi-kinase inhibitor targeting VEGFR and Raf kinases; and an unspecified programmed cell death protein 1 (PD-1) inhibitor. This combination aims to overcome resistance to prior immunotherapy by simultaneously targeting tumor angiogenesis, intracellular signaling pathways, and the immune microenvironment. It is currently being studied in a Phase II clinical trial sponsored by Tianjin Medical University Cancer Institute and Hospital as a second-line immune rechallenge therapy for patients who have failed previous PD-1/PD-L1 inhibitor treatments.
