Gemcitabine + nab-paclitaxel + S-1 (commonly known as the GAS or GeNeS1S regimen) is an investigational triple-combination chemotherapy regimen primarily utilized for the treatment of pancreatic ductal adenocarcinoma (PDAC), including locally advanced, metastatic, and borderline resectable cases. The regimen integrates three distinct cytotoxic mechanisms: gemcitabine, a nucleoside analog that inhibits DNA synthesis; nab-paclitaxel, a nanoparticle albumin-bound taxane that stabilizes microtubules to prevent cell division; and S-1, an oral fluoropyrimidine (comprising tegafur, gimeracil, and oteracil potassium) that inhibits thymidylate synthase. This combination has been extensively studied in Phase I and II clinical trials in Japan and China, demonstrating significant objective response rates and potential for conversion surgery in patients with advanced pancreatic malignancies.
