Overview

Guselkumab + JAK inhibitors is a combination therapy regimen being investigated for the treatment of refractory inflammatory bowel disease (IBD). This dual-targeted approach combines guselkumab, a human monoclonal antibody that selectively binds to the p19 subunit of interleukin-23 (IL-23), with a Janus kinase (JAK) inhibitor such as upadacitinib or tofacitinib. Guselkumab inhibits the IL-23 pathway, which is a key driver of chronic intestinal inflammation, while JAK inhibitors block the signaling of multiple pro-inflammatory cytokines by inhibiting Janus kinase enzymes (JAK1, JAK2, JAK3, and/or TYK2). The combination is primarily being explored in patients who have failed conventional biologics and small molecule therapies, aiming to achieve synergistic clinical and endoscopic improvements by simultaneously addressing multiple inflammatory pathways.
