This is a combination regimen consisting of three broad-spectrum antibiotics: - **Imipenem-cilastatin** is a fixed-dose combination of imipenem, a carbapenem antibiotic that inhibits bacterial cell wall synthesis, and cilastatin, an inhibitor of renal dehydropeptidase I that prevents the degradation of imipenem in the kidneys. - **Moxifloxacin** is a fluoroquinolone antibiotic that inhibits bacterial DNA gyrase and topoisomerase IV, enzymes essential for DNA replication and repair. - **Piperacillin-tazobactam** combines piperacillin, an extended-spectrum penicillin that inhibits bacterial cell wall synthesis, with tazobactam, a beta-lactamase inhibitor that protects piperacillin from enzymatic degradation by beta-lactamases. This triple-antibiotic regimen would provide extremely broad antibacterial coverage against Gram-positive bacteria (including some resistant strains), Gram-negative bacteria (including Pseudomonas aeruginosa), anaerobic organisms, and atypical pathogens. Such combinations are not standard clinical practice due to overlapping spectra and increased risk for toxicity but may be considered in rare cases involving highly resistant or polymicrobial infections where maximal empiric coverage is temporarily required[2][8].
