IOA-244 + ruxolitinib is a combination therapy under clinical investigation for the treatment of myelofibrosis (MF) and other hematologic malignancies in patients unresponsive to JAK inhibitors. IOA-244 (roginolisib) is a highly selective, non-ATP-competitive allosteric inhibitor of phosphoinositide 3-kinase delta (PI3Kδ), designed to modulate tumor immune responses, alter the tumor microenvironment, and directly inhibit PI3Kδ signaling relevant to cancer and autoimmune pathology[5][7]. Ruxolitinib is an oral small molecule inhibitor of Janus kinase 1 and 2 (JAK1/2), which disrupts cytokine-mediated signal transduction central to cell proliferation and survival in myeloproliferative neoplasms[2][6]. The combination is being researched to address resistance to JAK inhibitors and achieve enhanced disease modification in MF and potentially in other advanced malignancies[9].
