Overview

This is a combination immunosuppressive regimen consisting of three agents—methylprednisolone, mycophenolate mofetil, and tacrolimus—commonly used in solid organ transplantation, particularly kidney transplantation.

  • Methylprednisolone is a synthetic glucocorticoid corticosteroid that suppresses inflammation and modulates immune responses by binding to the glucocorticoid receptor, inhibiting multiple inflammatory cytokines and mediators.
  • Mycophenolate mofetil is an antimetabolite immunosuppressant that inhibits inosine monophosphate dehydrogenase (IMPDH), thereby blocking de novo guanosine nucleotide synthesis in T and B lymphocytes, leading to reduced proliferation of these immune cells.
  • Tacrolimus is a calcineurin inhibitor that binds to FK506-binding protein (FKBP12), inhibiting calcineurin phosphatase activity; this prevents activation of T-cell transcription factors such as NFAT (nuclear factor of activated T-cells) required for interleukin-2 gene expression and subsequent T-cell activation.

The triple combination aims to provide synergistic immunosuppression with non-overlapping mechanisms, reducing the risk of acute rejection after organ transplantation while allowing for lower doses of each drug to minimize toxicity. Clinical studies have shown this regimen results in excellent patient and graft survival rates with lower incidence of acute rejection compared to dual therapy regimens. The most common indications are prevention and treatment of organ transplant rejection.
