N-acetylcysteine + pentoxifylline is a fixed-dose combination of two small molecule drugs: N-acetylcysteine (NAC), a well-known antioxidant and glutathione precursor, and pentoxifylline (PTX), a methylxanthine derivative with anti-inflammatory, hemorheological, and vasodilatory properties. The combination is designed to target oxidative stress and inflammation in various disease states. NAC acts primarily by replenishing intracellular glutathione stores, reducing oxidative damage, while pentoxifylline inhibits the production of pro-inflammatory cytokines such as TNF-alpha and improves microcirculatory blood flow by decreasing blood viscosity[1][2][3][4]. This drug combination has been studied for its potential to ameliorate organ injury due to ischemia-reperfusion events (such as in liver or limb ischemia), reduce hepatic encephalopathy in severe alcoholic hepatitis, and protect against remote organ damage including renal or lung injury following systemic inflammatory insults[1][2][3][4][5]. Clinical studies suggest that while both agents are protective individually against tissue injury from oxidative stress or inflammation, their combined use may offer additive benefits for certain endpoints but not necessarily superior efficacy over monotherapy for all outcomes[5].
