Overview

The combination of nivolumab and prednisolone is used in cancer treatment. Nivolumab (brand name OPDIVO) is a monoclonal antibody that targets the PD-1 receptor, blocking it to restore the immune system's ability to recognize and attack cancer cells. Prednisolone is a corticosteroid primarily used in this combination to manage immune-related adverse events (irAEs) that can arise from immunotherapy, and also as part of specific treatment protocols. For severe (Grade 3) irAEs, nivolumab is typically withheld and corticosteroids are administered. For life-threatening (Grade 4) reactions, nivolumab is permanently discontinued. Treatment may resume after symptom resolution (Grade 0 to 1) and corticosteroid taper, but nivolumab is permanently discontinued if complete or partial resolution does not occur within 12 weeks, or if the corticosteroid dose cannot be reduced to 10 mg/day or less of prednisone (or equivalent) within 12 weeks. Research suggests that low-dose baseline corticosteroids (e.g., approximately 10mg/day prednisone equivalent) for inflammatory conditions may not compromise anti-tumor efficacy, but higher doses given for cancer-related palliative indications, especially within 24 hours of immunotherapy initiation, may be associated with poorer clinical outcomes.
