Overview

**nuvisertib + ruxolitinib** is an investigational, oral combination therapy studied for the treatment of *myelofibrosis* (including primary myelofibrosis and post–polycythemia vera or post–essential thrombocythemia myelofibrosis). Nuvisertib (TP-3654) is a highly selective **PIM1 kinase inhibitor**, while ruxolitinib is a widely approved **Janus kinase (JAK) 1/2 inhibitor**. The combination targets both the JAK/STAT and PIM1 pathways, respectively, which are implicated in the pathogenesis, resistance, and progression of myelofibrosis. Clinical studies show the combination leads to reduction in spleen volume, improvement in symptoms and hematologic parameters (including normalization of white blood cell/neutrophil counts and improvement in anemia), and reduction in bone marrow fibrosis. Nuvisertib is being developed by Sumitomo Pharma; ruxolitinib is developed by Incyte. Trials show the combination is generally well tolerated with manageable adverse events, and aims to overcome disease progression and treatment resistance in myelofibrosis that often limits the long-term efficacy of JAK inhibitors alone[1][2][3][4][5][6][7][8].
