Overview

The combination of prednisone (a corticosteroid) and vedolizumab (a monoclonal antibody) is primarily used for treating inflammatory bowel diseases (IBD), specifically ulcerative colitis (UC) and Crohn's disease. This combination therapy has shown significant benefits for patients. In ulcerative colitis, a study indicated that vedolizumab combined with rapid prednisolone (PSL) induction therapy significantly improves outcomes, with higher clinical remission rates at week 8 for patients receiving at least 20 mg of prednisolone within one week of vedolizumab induction (85.7% vs. 37.5%). The clinical activity index (CAI) also showed superior improvement in the combination group. C-reactive protein (CRP) levels at week 2 after vedolizumab induction may serve as a biomarker influencing continuation rates. For Crohn's disease, adding vedolizumab to stable doses of corticosteroids can help induce clinical response or remission, with clinical trials showing higher remission rates for the combination versus placebo. Vedolizumab is a recombinant humanized IgG1 monoclonal antibody that targets the human lymphocyte α4β7 integrin, blocking its interaction with MAdCAM1 to reduce lymphocyte migration to the gut and decrease inflammation. Prednisone is a corticosteroid that suppresses inflammation through multiple mechanisms. The combination appears to be safe, with adverse event rates similar to monotherapy. No specific drug interactions between vedolizumab (Entyvio) and prednisone have been reported. Clinical applications include induction therapy for moderate to severe ulcerative colitis, improving clinical response in Crohn's disease patients not fully responsive to steroids, and potentially providing a steroid-sparing effect long-term. This combination may be more effective than either drug alone in inducing remission in active IBD.
