This combination consists of two pharmaceutical agents: prednisone, a synthetic glucocorticoid corticosteroid, and tofacitinib, an oral small molecule Janus kinase (JAK) inhibitor. Prednisone acts as a prodrug that is converted in the liver to prednisolone, which binds the glucocorticoid receptor and modulates gene expression leading to anti-inflammatory and immunosuppressive effects. Tofacitinib inhibits JAK enzymes involved in cytokine signaling pathways critical for immune cell function and inflammation. The combination may be used off-label or in clinical practice for autoimmune diseases such as rheumatoid arthritis or ulcerative colitis when monotherapy is insufficient. However, combining these drugs increases the risk of serious infections due to additive immunosuppression[1][2][7]. Both drugs are approved individually but not as a fixed-dose combination.
