RiboPGT121-LS is an mRNA-based therapeutic candidate developed by BioNTech for the treatment and prevention of HIV-1 infection. It consists of mRNA encoding the broadly neutralizing antibody (bNAb) PGT121, which has been modified with "LS" mutations (M428L/N434S) in the Fc region to extend its half-life via increased affinity for the neonatal Fc receptor (FcRn). The mRNA is encapsulated in lipid nanoparticles (LNPs) for delivery. Upon administration, the mRNA is translated by host cells to produce and assemble the functional PGT121-LS antibody in vivo. This approach aims to circumvent the manufacturing complexities associated with recombinant protein production while maintaining the potent viral neutralization and pharmacokinetic profile of the original antibody. Preclinical studies in non-human primates have demonstrated that intravenous administration is well-tolerated and achieves therapeutic antibody titers comparable to recombinant protein delivery.
