Ruxolitinib + sabatolimab is an investigational combination therapy used in clinical studies for patients with myelofibrosis who have a suboptimal response to ruxolitinib alone. Ruxolitinib is a small molecule inhibitor of Janus kinase 1 and Janus kinase 2 (JAK1/JAK2), and is the current standard of care for symptomatic myelofibrosis. Sabatolimab is a monoclonal antibody that targets T-cell immunoglobulin and mucin domain-3 (TIM-3), an immune checkpoint molecule expressed on myeloproliferative neoplasm (MPN) progenitor cells. The rationale for the combination is that ruxolitinib suppresses pro-inflammatory cytokine signaling and pathological hematopoietic cell proliferation, while sabatolimab is designed to augment anti-tumor immunity via TIM-3 blockade, potentially improving disease control in MPNs. The combination was studied in the ADORE (NCT04097821) Phase 1b/2 open-label platform trial[2].
