Sivelestat sodium hydrate + ulinastatin is a combination therapy of two protease inhibitors used primarily in the management of sepsis with acute respiratory distress syndrome (ARDS) and other forms of severe lung injury. Sivelestat sodium hydrate is a selective neutrophil elastase inhibitor that reduces inflammation and tissue damage by blocking neutrophil elastase activity, thereby mitigating lung injury and improving pulmonary function. Ulinastatin is a broad-spectrum serine protease inhibitor that suppresses inflammatory responses by inhibiting various enzymes such as trypsin, chymotrypsin, elastase, and plasmin. The combination has been shown to improve clinical outcomes in patients with sepsis-induced ARDS by enhancing lung function, reducing inflammation and oxidative stress markers, shortening mechanical ventilation time and ICU stay, and lowering the incidence of multiple organ dysfunction syndrome[1][2]. Both agents act synergistically to control excessive inflammatory cascades associated with critical illness.
