Overview

TNF inhibitors + DMARDs refers to the combination therapy of tumor necrosis factor (TNF) inhibitor drugs with disease-modifying anti-rheumatic drugs (DMARDs). This regimen is commonly used in the management of autoimmune inflammatory diseases such as rheumatoid arthritis, psoriatic arthritis, and other related conditions. TNF inhibitors are biologic agents that specifically block the activity of tumor necrosis factor-alpha, a cytokine central to inflammation. Common TNF inhibitors include infliximab, adalimumab, etanercept, golimumab, and certolizumab. DMARDs are a broader class that includes conventional synthetic agents like methotrexate, sulfasalazine, hydroxychloroquine, leflunomide; targeted synthetic agents such as JAK inhibitors; and other biologics. The rationale for combining these therapies is to achieve better disease control than with either agent alone—biologic DMARDs (such as TNF inhibitors) are often most effective when paired with a conventional synthetic DMARD like methotrexate. The combination can reduce symptoms (pain, swelling), slow or prevent joint damage/progression of disease by targeting different aspects of immune dysregulation[1][3][4][5]. This approach is standard for patients who have not responded adequately to monotherapy.
