Tofacitinib + triamcinolone is a combination therapy being evaluated in a Phase 4 clinical trial by the All India Institute of Medical Sciences (AIIMS), Bhubaneswar, for the treatment of moderate to severe oral lichen planus (OLP). The regimen consists of oral tofacitinib, a small molecule Janus kinase (JAK) inhibitor that selectively targets JAK1 and JAK3 to interrupt the JAK-STAT signaling pathway, and topical triamcinolone, a potent corticosteroid that acts as a glucocorticoid receptor agonist. This dual approach aims to provide superior anti-inflammatory and immunomodulatory effects compared to standard topical steroid monotherapy, particularly in patients with erosive OLP who are resistant to conventional treatments or experience frequent relapses.
