Overview

This is a combination chemotherapy regimen consisting of five agents: 5-fluorouracil, cisplatin, dexrazoxane, doxorubicin, and vincristine. Each component has a distinct mechanism of action targeting cancer cells:

  • 5-fluorouracil is an antimetabolite that inhibits thymidylate synthase, blocking DNA synthesis.
  • Cisplatin is a platinum-based compound that forms DNA crosslinks and induces apoptosis.
  • Dexrazoxane acts as a cardioprotective agent by chelating iron and reducing free radical formation; it is often used to mitigate the cardiotoxicity associated with anthracyclines like doxorubicin.
  • Doxorubicin is an anthracycline antibiotic that intercalates into DNA and inhibits topoisomerase II, leading to DNA damage.
  • Vincristine disrupts microtubule formation during mitosis.

This multi-agent regimen aims to maximize anti-tumor efficacy by combining drugs with complementary mechanisms. Such regimens are primarily used in pediatric oncology for conditions like hepatoblastoma or other solid tumors where aggressive multi-drug therapy may be indicated. The addition of dexrazoxane specifically helps reduce the risk of cardiac toxicity from doxorubicin.
