Overview

The combination of ascorbic acid (vitamin C), hydrocortisone, and thiamine—commonly referred to as "HAT therapy"—is an investigational intravenous regimen primarily studied for the treatment of sepsis and septic shock. The rationale for this combination is based on the potential synergistic effects of each component in modulating inflammation, supporting adrenal function, reducing oxidative stress, and improving cellular metabolism during critical illness. Ascorbic acid acts as an antioxidant and may reduce endothelial injury; hydrocortisone is a corticosteroid that modulates inflammatory responses; thiamine (vitamin B1) is essential for cellular energy metabolism. While early observational studies suggested significant reductions in organ dysfunction and mortality with HAT therapy in sepsis patients, subsequent randomized controlled trials have produced mixed results regarding its efficacy on clinical outcomes such as organ failure scores or mortality[1][2][5]. The regimen has not been universally adopted into standard care guidelines.
