Baitouweng decoction + mesalamine is a combination therapy consisting of Baitouweng decoction—a traditional Chinese herbal formula—and mesalamine, a standard anti-inflammatory drug used in the management of inflammatory bowel diseases. Baitouweng decoction (also known as Bai Tou Weng Tang) is composed primarily of Pulsatilla chinensis, Coptis chinensis, Phellodendron chinense, and Fraxinus chinensis. It has been traditionally used to treat gut-related disorders such as ulcerative colitis (UC), with demonstrated effects including anti-inflammatory action, modulation of cytokines (e.g., reducing TNF-α, IL-1β, IL-6; increasing IL-10), restoration of intestinal barrier function via tight junction protein expression enhancement, and regulation of tryptophan metabolism through the aryl hydrocarbon receptor pathway[4][6][8]. Mesalamine is a small molecule aminosalicylate that acts locally in the colon to inhibit cyclooxygenase and reduce prostaglandin production—thereby decreasing inflammation. The combination has shown synergistic effects in preclinical models and clinical studies for ulcerative colitis and ulcerative proctitis. The combined regimen more effectively reduces disease activity index scores and inflammatory markers compared to either agent alone[2][3][5]. Mechanistically, this synergy may be due to complementary actions on inflammatory pathways (including inhibition of nuclear factor kappa B p65 activation) and improved repair of mucosal barriers.
