BCMA-targeted CAR T cells + dexamethasone + NT-I7 is an investigational combination therapy being evaluated for the treatment of relapsed and refractory multiple myeloma. The regimen consists of ciltacabtagene autoleucel (a BCMA-directed CAR T-cell therapy), the corticosteroid dexamethasone, and NT-I7 (efineptakin alfa), a long-acting recombinant human interleukin-7 (IL-7) fusion protein. The therapeutic strategy utilizes dexamethasone to upregulate the expression of the IL-7 receptor alpha (IL-7Rα) on the surface of the CAR T cells, thereby sensitizing them to IL-7 signaling. Subsequent administration of NT-I7 leverages this increased receptor density to promote the expansion, metabolic fitness, and long-term persistence of the CAR T cells in the tumor microenvironment. This combination aims to overcome the common challenge of limited CAR T-cell durability and prevent early relapse in myeloma patients.
