Overview

This is a combination of five antibiotics—ciprofloxacin, clarithromycin, clofazimine, metronidazole, and rifabutin. Each component has a distinct mechanism of action targeting different bacterial processes:

  • Ciprofloxacin is a fluoroquinolone antibiotic that inhibits bacterial DNA gyrase and topoisomerase IV, enzymes essential for DNA replication.
  • Clarithromycin is a macrolide antibiotic that binds to the 50S ribosomal subunit and inhibits protein synthesis in susceptible bacteria.
  • Clofazimine is an antimycobacterial agent with anti-inflammatory properties; it binds to mycobacterial DNA and disrupts template function.
  • Metronidazole is a nitroimidazole antibiotic effective against anaerobic bacteria and protozoa by causing DNA strand breakage after reduction within the cell.
  • Rifabutin is a rifamycin derivative that inhibits bacterial DNA-dependent RNA polymerase.

This combination would provide broad-spectrum antibacterial activity against aerobic Gram-negative organisms (ciprofloxacin), Gram-positive cocci (clarithromycin), mycobacteria (clofazimine, rifabutin), anaerobes (metronidazole), and atypical pathogens. Such combinations are sometimes considered in complex or resistant infections or for specific indications like multidrug-resistant tuberculosis or nontuberculous mycobacterial infections. However, there are significant risks of drug-drug interactions—especially QT prolongation—and overlapping toxicities.
