This is a combination regimen consisting of four drugs: - Cyclophosphamide, an alkylating agent that damages DNA and prevents cancer cell replication. - Docetaxel, a taxane that disrupts microtubule function, inhibiting cell division. - Doxorubicin, an anthracycline antibiotic that intercalates DNA and inhibits topoisomerase II, leading to DNA damage and apoptosis. - Zoledronic acid, a bisphosphonate that inhibits osteoclast-mediated bone resorption. The combination of cyclophosphamide, docetaxel, and doxorubicin (often referred to as TAC) is used as first-line chemotherapy for breast cancer—especially metastatic or high-risk early-stage disease—due to its high response rates[1][3][5]. Zoledronic acid is added in patients with bone metastases or at risk for skeletal-related events; it reduces the risk of fractures and other complications by strengthening bones[6][10]. The overall mechanism involves direct cytotoxicity against tumor cells (by the three chemotherapeutic agents) combined with protection against bone complications from malignancy (by zoledronic acid).
