Overview

This is a combination of four disease-modifying therapies (DMTs) used in the treatment of multiple sclerosis (MS): dimethyl fumarate, glatiramer acetate, interferon beta, and teriflunomide. Each agent has a distinct mechanism of action targeting immune modulation to reduce MS relapses and slow disease progression. Dimethyl fumarate is an oral small molecule that activates the Nrf2 pathway to exert anti-inflammatory and cytoprotective effects. Glatiramer acetate is a synthetic polypeptide that mimics myelin basic protein, modulating T-cell responses toward an anti-inflammatory profile. Interferon beta is a biologic immunomodulator that reduces proinflammatory cytokine production and increases anti-inflammatory cytokines via activation of the interferon receptor on leukocytes. Teriflunomide is an oral immunomodulatory agent that inhibits dihydroorotate dehydrogenase, reducing proliferation of activated lymphocytes. While all four drugs are approved as monotherapies for relapsing forms of MS, there are no established clinical indications or approvals for their combined use as a single regimen; combination DMT with these agents is not standard practice due to safety concerns and lack of evidence for additive benefit[1][2][8].
