Ethosuximide + valproate is a combination of two antiepileptic drugs used primarily in the management of absence seizures, especially in patients who do not respond adequately to monotherapy. Ethosuximide is a succinimide anticonvulsant that acts mainly by blocking T-type voltage-gated calcium channels in thalamic neurons, thereby suppressing the abnormal electrical activity associated with absence seizures. Valproate (valproic acid) is a broad-spectrum antiepileptic drug that exerts its effects through multiple mechanisms, including increasing brain levels of gamma-aminobutyric acid (GABA), inhibiting voltage-gated sodium channels, and possibly modulating T-type calcium channels. The combination has been shown to have additive or synergistic anticonvulsant effects and may offer an improved efficacy-to-toxicity ratio compared to either drug alone[1][5]. This combination is particularly considered for atypical and myoclonic absences or when monotherapy fails[1][3].
