Overview

This is a combination therapy consisting of three drugs—**infliximab**, **mesalazine** (also known as mesalamine), and **pentoxifylline**—used in the management of severe, refractory inflammatory conditions such as ulcerative colitis and associated extraintestinal manifestations like pyoderma gangrenosum. - **Infliximab** is a chimeric monoclonal antibody that targets tumor necrosis factor-alpha (TNF-α), a key pro-inflammatory cytokine, thereby reducing inflammation by blocking TNF-α signaling pathways[2]. - **Mesalazine** is an aminosalicylate anti-inflammatory agent that acts locally in the colon to inhibit prostaglandin and leukotriene synthesis, reduce leukocyte migration, and scavenge free radicals[9]. - **Pentoxifylline** is a methylxanthine derivative with immunomodulatory properties; it inhibits phosphodiesterase activity leading to increased cAMP levels, reduces production of pro-inflammatory cytokines (including TNF-α), decreases C-reactive protein levels, and inhibits Nuclear Factor-kappa Beta (NF-kB) transcription involved in inflammatory gene expression[1]. This triple combination has been reported in case studies for patients with ulcerative colitis who are refractory to standard therapies. The rationale for combining these agents lies in their complementary mechanisms targeting different aspects of the inflammatory cascade.
