This combination regimen is a multi-agent therapeutic strategy being evaluated for the second-line treatment of metastatic colorectal cancer (mCRC). It integrates a cytotoxic chemotherapy backbone—consisting of a nanoliposomal formulation of the topoisomerase I inhibitor irinotecan, the antimetabolite 5-fluorouracil (5-FU), and the biochemical modulator leucovorin—with the anti-angiogenic monoclonal antibody bevacizumab and the novel bifunctional antibody enlonstobart (QL1706). Enlonstobart is a 'Mab-body' that simultaneously targets the immune checkpoints programmed cell death protein 1 (PD-1) and cytotoxic T-lymphocyte-associated protein 4 (CTLA-4). By combining cytotoxic, anti-angiogenic, and dual-checkpoint inhibitory mechanisms, the regimen aims to overcome tumor resistance and enhance anti-tumor immune responses in patients who have progressed on prior therapies. It is currently under investigation in the Phase II/III IRIS-02 clinical trial.
